Permission & Agreement for Ethio-United Soccer Academy (EUSA)
I, [Parent/Guardian Name], hereby grant permission for my child, [Child’s Name], to participate in all activities and programs offered by Ethio-United Soccer Academy (EUSA). I understand that EUSA, in partnership with Lens Sport, Barudy Club, and Stallions Sport Academy, provides soccer training and related activities for children aged 4 to 16.
Media Release:
I grant EUSA and any associated organizations permission to photograph and videotape my child during their participation in EUSA activities. This includes, but is not limited to, training sessions, matches, tournaments, and other events.
I further consent to EUSA using my child’s likeness for promotional purposes. This may include, without limitation, publishing images on the organization’s website, social media, or other media platforms.
I understand that my child’s first name, jersey number, and position may be used in conjunction with their photo on the EUSA website, club website, team websites, and other commercial or social media platforms.
Liability Waiver:
While EUSA takes precautions to ensure the safety of all participants, I acknowledge that there are inherent risks associated with sports activities. I agree to release and hold harmless EUSA, its coaches, staff, partners, and volunteers from any and all liability for any injury or loss that my child may sustain while participating in EUSA activities, except to the extent such injury or loss is caused by the gross negligence or willful misconduct of EUSA or its agents.
Medical Consent:
In the event of a medical emergency, I authorize EUSA to obtain necessary medical treatment for my child. I will be responsible for all associated medical costs.
